Saturday sessions and those over 90 minutes must call to book.
Please arrive 10 minutes early if this is your initial visit to complete the necessary forms if you have not already done so online through the scheduling website. If you prefer, you may go to the Client Forms tab, print, and hand write that version. The more thorough you are, the better I know to serve you. And feel free to fill in your age as I do send out birthday specials that are good during your birthday month
Clients with medical, wellness or auto insurance benefitsmust provide full name, DOB, name of insurance company, member ID and group number to verify benefits and preferred provider status prior to appointment. Please allow plenty of time for the verification process when considering your appointment date. Normally 3-5 days is sufficient. Appointments billed under medical insurance for acute or chronic injury, or under auto insurance must be accompanied by a written prescription for massage therapy from a medical doctor, chiropractor, or physical therapist. If you are unsure and this is your first appointment under insurance billing, please call and we can set up the scheduling together to be sure everything is properly coordinated for you to get the most of your massage session. Copays will be due at time of session.
Cancellation Policy I understand that unanticipated events happen occasionally in everyone’s life. In my desire to be effective and fair to all clients, the following policies are honored:
•24 hour advance notice is requested when cancelling an appointment. This allows the opportunity for someone else to schedule an appointment. If after two incidences that you are unable to give us 24 hours advance notice, you will be charged the full amount of your next scheduled appointment. This amount must be paid prior to the day of your next scheduled appointment.
Massage work cannot be performed to clients experiencing cold, flu or viral symptoms. Under this circumstances the appointment will be rescheduled and the late cancellation fee will be waived. •No-shows Anyone who either forgets or consciously chooses to forgo their appointment for whatever reason will be considered a “no-show.” After the first no-show, clients will be required to submit a deposit for the full cost of the next massage when scheduling future appointments.
•Late Arrivals If you arrive late, your session may be shortened in order to accommodate others whose appointments follow yours. Depending upon how late you arrive, your therapist will then determine if there is enough time remaining to start a treatment. Regardless of the length of the treatment actually given, you will be responsible for payment of the “full” session. Out of respect and consideration to your therapist and other customers, please plan accordingly and be on time.
•Massage Therapist Cancellation Should I need to cancel a client's massage within 24 hours advanced notice, the client will receive $10 off their next massage.
